Doc enhancements to upgrade.sh:
* Docs make it more clear that p4d is expected to be online at the start of processing.
* Enhanced error message for scenario where a cautious admin stops p4d manually first,
causing the script to abort the upgrade.
* Verified that the call to 'verify_sdp.sh' is captured in the output; this is done by
verify_sdp.sh itself.
